We appreciate your interest in Children & Teen Dental of Florida! Our office asks that you please print the form attached below and fill it out to bring to your child’s scheduled appointment. For those without a printer, please arrive at our office at least 30 minutes in advance of the scheduled appointment to fill out the forms.
